Your stage helps us tailor protocol intensity and focus. If you're unsure, check your pathology report or select "Unknown" - we may find this in your uploaded medical records.
Select all locations where cancer has spread. Metastasis locations determine organ-specific supplement strategies. If you're unsure, check your imaging reports or select "Unknown" - we can find this in your uploaded medical records.
Select timeframe
This helps us understand your treatment timeline and urgency level
List all genetic or biomarker test results from your pathology or genomic testing (examples: KRAS, HER2+, EGFR, BRCA1/2, TP53, PD-L1 positive, MSI-High, BRAF, ALK, ROS1, NRAS, PIK3CA, etc.). These are CRITICAL for targeted supplement recommendations. If you don't know or haven't been tested, type "Unknown" or "Not tested" - we can often find this information in your uploaded pathology report.
List any tumor marker test results with numbers (examples: CEA, CA 19-9, PSA, CA-125, AFP, CA 15-3, CA 27-29, etc.). Include the number and date if possible (e.g., "CEA: 45 on 12/1/24"). Tumor markers help us track protocol effectiveness over time. If you don't know or haven't had these tested, type "Unknown" - we can find this in your uploaded bloodwork.
Select your current status
This determines which supplements are safe to take RIGHT NOW. Some supplements can't be taken during active chemotherapy, while others enhance treatment effectiveness.
Select all that apply, including past and current treatments. This helps us avoid dangerous interactions, support treatment effectiveness, and address treatment side effects.
Select clinical trial status

List every medication you're currently taking, including chemotherapy drugs, targeted therapies, immunotherapy, and medications for other conditions (blood pressure, diabetes, thyroid, etc.). Include dosages and frequency (e.g., "Tamoxifen 20mg daily", "Lisinopril 10mg daily"). This prevents dangerous supplement-drug interactions. If you don't remember exact dosages, list what you can - we may find details in your uploaded medical records.
List every supplement, vitamin, mineral, herb, or natural product you're currently taking. Include dosages and frequency (e.g., "Fish Oil 2000mg daily", "Turkey Tail mushroom extract 1g twice daily", "CBD oil 25mg"). This prevents duplicate recommendations and checks for interactions with your medications. If you're not taking any, type "None".

List any treatments that didn't work or stopped working (e.g., "Keytruda - no response after 6 months", "Radiation - tumor regrew", "FOLFOX - disease progressed"). Knowing what DIDN'T work helps us understand treatment resistance patterns and adjust protocol strategies. If none or not applicable, type "N/A".
Select recurrence status
Recurrence history changes our protocol focus to root cause elimination and immune system rebuilding to prevent future recurrence.
Enter your oncologist's name and where they practice. This helps us understand your treatment team and coordinate care if needed. We won't contact them without your permission. If you don't have an oncologist yet, type "I don't have an oncologist yet".
Select doctor's openness level
This helps us frame recommendations in a way your doctor will accept and understand. We can provide research citations for skeptical doctors.
Select functional ability
Your functional status (also called ECOG or performance status) determines protocol intensity and realistic goals. Be honest - this helps us design a protocol you can actually follow.
Your current weight is used for supplement dosing calculations and nutritional planning. Approximate weight is fine.
Select weight change
Significant weight changes (especially unintentional weight loss) indicate metabolic shifts we need to address with nutrition and supplement strategies

Direct sun exposure (without sunscreen) on bare skin affects Vitamin D production, which is critical for immune function and cancer prevention. We'll include Vitamin D supplementation if you're not getting enough sun.
Select diet type
Your current diet is our starting point for nutrition recommendations. We'll build on what you're already doing rather than asking you to change everything at once.
Select water intake
Daily water intake (not including coffee, tea, or other beverages). Proper hydration is critical for detoxification, nutrient absorption, and healing.

List any family members (parents, siblings, grandparents, aunts/uncles) who had cancer, including cancer type and age at diagnosis if known (e.g., "Mother - breast cancer at 55", "Father - colon cancer at 62"). Family history helps identify genetic risk factors and guide prevention strategies for recurrence. If no family history, type "None".
Select caregiver status
